Clear aligners are evolving… but is the evidence keeping up?

Clear aligner therapy has become one of the most widely discussed areas of modern orthodontics.

With millions of patients being treated annually, aligners are now being used not only for mild malocclusions, but increasingly for more complex orthodontic tooth movements. As aligners continue expanding into more advanced treatment planning, clinicians are asking important questions around predictability, biomechanics, efficiency, and long-term biological outcomes.

A recently published narrative review aimed to bring together the current evidence surrounding clear aligner therapy into one clinically relevant overview.

Rather than focusing on a single aspect of treatment, the paper examines biomechanics, treatment efficiency, extraction therapy, attachment design, movement predictability, deep bite correction, distalisation, and biological considerations including microplastic release and biocompatibility.

Our Summary

This narrative review examined a wide range of evidence surrounding clear aligner therapy (CAT), including biomechanics, treatment efficiency, extraction mechanics, distalisation, deep bite correction, attachment design, movement predictability, and biological considerations.

The paper highlights several well-established advantages of aligners, including reduced chairside time, fewer emergency appointments, improved aesthetics, and favourable short-term oral hygiene outcomes compared to fixed appliances.

Patients also tend to report lower levels of pain and treatment-related anxiety during the early stages of treatment.

At the same time, the review emphasises that certain movements remain less predictable with aligners, particularly extrusion, posterior derotation, translation, and torque control.

The Findings

The paper highlights that extraction cases treated with aligners may take approximately six months longer than comparable fixed appliance cases.

It also discusses how aligners may produce unwanted movements during extraction treatment, including incisor extrusion, distal tipping of canines, and mesial tipping of molars.

One particularly useful section focuses on attachment design and biomechanics. The review summarises how different attachment configurations may improve aligner retention, anchorage, force delivery, and movement predictability depending on the clinical objective.

The authors also reviewed current evidence surrounding microplastic release from aligners. While release does occur, there is currently no evidence demonstrating adverse clinical effects.

Importantly, the paper repeatedly highlights the need for more robust long-term evidence across many areas of aligner therapy.

Clinical Interpretation

One of the strengths of this review is its balanced approach.

Clear aligners are often discussed in extremes, either as a complete replacement for fixed appliances or as inherently limited in complex treatment. This paper avoids both positions and instead examines where evidence currently supports aligner therapy, where limitations remain, and where uncertainty still exists.

For clinicians, the review reinforces an important point: successful aligner treatment depends heavily on understanding biomechanics rather than relying solely on software-driven planning.

The discussion around attachment design, staging strategies, and extraction mechanics is particularly valuable because it highlights how aligner biomechanics differ fundamentally from conventional fixed appliance systems.

At the same time, the paper also reinforces that many clinical protocols within aligner therapy continue evolving faster than the long-term evidence base itself.

Study Limitation

As a narrative review, the paper summarises and interprets existing evidence rather than presenting new clinical trial data. Some conclusions therefore remain dependent on the quality and consistency of the available literature.
